ABSTRACT

O objetivo deste estudo é descrever o caso de mulher com síndrome de Meigs e apresentar a revisão narrativa sobre o tema. Paciente do sexo feminino, 30 anos, nulípara, encaminhada ao hospital por massa anexial e história prévia de drenagem de derrame pleural. Evoluiu com instabilidade hemodinâmica por derrame pleural hipertensivo à direita, sendo submetida a drenagem torácica, com citologia do líquido negativa. Após, foi submetida a laparotomia: realizada salpingo-ooforectomia esquerda. A congelação e a análise histopatológica diagnosticaram fibroma ovariano. A citologia ascítica foi negativa. CA-125 elevado, presença de derrames cavitários e exame de imagem suspeito podem mimetizar um cenário de neoplasia maligna de ovário em estágio avançado. Entretanto, na síndrome de Meigs clássica, o tratamento é cirúrgico, sendo o diagnóstico obtido por meio da análise histopatológica do tumor ovariano. O manejo da síndrome de Meigs clássica é cirúrgico e, após a remoção do tumor, o derrame pleural e a ascite desaparecem.


To describe a case of Meigs syndrome and present a narrative review of the condition. Female patient, 30 years old, nulliparous, referred to the hospital due to an adnexal mass and a previous drainage of pleural effusion. She developed hemodynamic instability due to a hypertensive right pleural effusion being submitted to chest drainage, with negative cytology of the fluid. She underwent laparotomy: Left salpingo-oophorectomy was performed and frozen section and histopathological analysis diagnosed an ovarian fibroma. Ascites cytology was negative. Elevated CA-125, presence of cavitary effusions, suspicious imaging exam can mimic a scenario of ovarian cancer at an advanced stage. However, in classical Meigs syndrome, treatment is surgical, and the diagnosis is obtained through histopathological analysis of the ovarian tumor. Classical Meigs syndrome' management is surgical. After tumor removal, pleural effusion and ascites resolve.

ABSTRACT

Cough may be associated with complications such as syncope, urinary incontinence, pneumothorax, and less frequently, pulmonary hernia and costal fractures. Chronic cough is a cause of rib fractures and when they occur it is likely to affect more than one rib. We report a 53 year-old obese male in treatment with enalapril 10 mg for hypertension with a dry cough lasting five months. He consulted for bilateral chest pain and a Chest X ray examination showed symmetrical fractures in the seventh left and right ribs. Enalapril was discontinued, cough and pain subsided in two weeks.

ABSTRACT CONTEXT AND OBJECTIVE: Post-thoracotomy pain is a severe and intense pain caused by trauma to ribs, muscles and peripheral nerves. The current study aimed to compare subpleural analgesia (SPA) with thoracic epidural analgesia (TEA) in patients undergoing thoracotomy. DESIGN AND SETTING: Randomized study at Ankara Numune Education and Research Hospital, in Turkey. METHODS: Thirty patients presenting American Society of Anesthesiologists physical status I-III were scheduled for elective diagnostic thoracotomy. The patients were randomized to receive either patient-controlled SPA or patient-controlled TEA for post-thoracotomy pain control over a 24-hour period. The two groups received a mixture of 3 µg/ml fentanyl along with 0.05% bupivacaine solution through a patient-controlled analgesia pump. Rescue analgesia was administered intravenously, consisting of 100 mg tramadol in both groups. A visual analogue scale was used to assess pain at rest and during coughing over the course of 24 hours postoperatively. RESULTS: In the SPA group, all the patients required rescue analgesia, and five patients (33%) required rescue analgesia in the TEA group (P < 0.05). Patients who received subpleural analgesia exhibited higher visual analogue scores at rest and on coughing than patients who received thoracic epidural analgesia. None of the patients had any side-effects postoperatively, such as hypotension or respiratory depression. CONCLUSION: Thoracic epidural analgesia is superior to subpleural analgesia for relieving post-thoracotomy pain. We suggest that studies on effective drug dosages for providing subpleural analgesia are necessary.


RESUMO CONTEXTO E OBJETIVO: Dor pós-toracotomia é severa e intensa, causada por trauma de nervos periféricos, músculos e costelas. O objetivo foi comparar analgesia subpleural (SPA) com analgesia epidural torácica (TEA) em pacientes submetidos à toracotomia. TIPO DE ESTUDO E LOCAL: Estudo randomizado no Hospital Educação e Pesquisa de Numune, em Ancara, Turquia. MÉTODOS: Trinta pacientes com o estado físico I-III da Sociedade Americana de Anestesiologistas foram agendados para toracotomia diagnóstica eletiva e escolhidos aleatoriamente para receber, num período de 24 horas, SPA ou TEA, ambas controladas pelo próprio paciente, para controle da dor pós-toracotomia. Os dois grupos receberam mistura de 3 ug/ml de fentanil com solução de 0,05% de bupivacaína por meio de bomba de analgesia controlada pelo paciente. Foi administrada analgesia de resgate por via intravenosa, com 100 mg de tramadol, nos dois grupos. No pós-operatório, a escala visual analógica foi utilizada para medir presença de dor durante a tosse ou em repouso, ao longo de 24 horas. RESULTADOS: No grupo SPA, todos os pacientes necessitaram de analgesia de resgate. Cinco pacientes (33%) necessitaram de analgesia de resgate no grupo TEA (P < 0,05). Os pacientes com SPA exibiram pontuações superiores na escala visual analógica, em repouso e ao tossir, em comparação aos que receberam TEA. Nenhum dos pacientes teve quaisquer efeitos secundários no pós-operatório, como hipotensão ou depressão respiratória. CONCLUSÃO: A analgesia peridural torácica é superior à analgesia subpleural no alívio da dor pós-toracotomia. Consideramos que estudos sobre a dosagem de drogas eficazes para proporcionar analgesia subpleural são necessários.

ABSTRACT

Transdiaphragmatic intercostal hernia is uncommon and mostly related to blunt or penetrating trauma. We report three similar cases of cough-induced transdiaphragmatic intercostal hernia, highlighting the anatomic findings obtained with different imaging modalities (radiography, ultrasonography, CT, and magnetic resonance) in each of the cases.


Hérnias intercostais transdiafragmáticas são eventos raros e são geralmente relacionadas a traumas abertos ou fechados, com risco de complicações. Relatamos três casos semelhantes, decorrentes de crises de tosse, destacando o aspecto das alterações anatômicas nos exames de imagem obtidos em cada situação (radiografia, ultrassonografia, TC e ressonância magnética).

ABSTRACT

Relatar o caso de uma paciente com retinopatia por Valsalva que evoluiu com hemorragia submembrana limitante interna e foi submetida à vitrectomia via pars plana, tendo tido melhora da acuidade visual. Paciente hígida de 35 anos evoluiu com redução súbita e indolor da acuidade visual do olho direito, após crise de tosse. Ao exame oftalmológico, apresentava hemorragia pré-macular, sem outras alterações. Inicialmente, optou-se pela conduta expectante, porém não houve regressão completa da hemorragia. Então, foi indicada a vitrectomia via pars plana associada à remoção da membrana limitante interna, havendo melhora considerável da acuidade visual, sem complicações peri-operatórias e sem alterações significativas à tomografia de coerência óptica, autofluorescência ou eletrorretinograma multifocal. Neste caso, o tratamento da hemorragia sub-membrana limitante interna com a cirurgia vitreorretiniana resultou em melhora da acuidade visual e regressão da hemorragia.


To report a case of a patient with Valsalva retinopathy that developed sub-internal limiting membrane hemorrhage, underwent pars plana vitrectomy and had visual acuity improvement after that. A 35-year-old healthy patient presented with sudden and painless vision loss of her right eye, after coughing. During the ophthalmologic evaluation, she had a pre-macular hemorrhage and no other abnormalities. Initially, we chose for expectant management, but the hemorrhage did not clear totally. Thus, pars plana vitrectomy associated with internal limiting membrane peeling was indicated, with considerable improvement of her visual acuity, without perioperative complications or significant findings in the optical coherence tomography, autofluorescence and multifocal electroretinogram. In this case, sub-internal limiting membrane hemorrhage treatment with vitreoretinal surgery was relatively useful, with visual acuity improvement and resolution of sub-internal limiting membrane hemorrhage.

ABSTRACT

PURPOSE: SUI (Stress Urinary Incontinence) results from sudden increases in intravesical peak pressures exceeding urethral resistance leading to involuntary urine loss. Obesity and smoking are well established reversible risk factors for SUI and may alter intravesical peak pressures. BMI, smoking status, and other clinical factors were studied to determine their relationship to CIPP (maximal Intravesical Peak Pressures generated by Cough) in SUI complaining women. MATERIALS AND METHODS: Three hundred nineteen women complaining of SUI were evaluated with medical history and urodynamics. Age, parity, comorbidities, previous surgery, BMI and history of smoking were obtained. The maximal intravesical peak pressures generated by cough (CIPP) and cough leak point pressure (CLPP) were acquired. Univariate and multivariate analysis were conducted. RESULTS: Current smokers and former smokers had similar CIPP (170cmH2O and 170cmH2O; p = 0.5, respectively); Those individuals who had never smoked had significantly lower CIPP (140cmH2O; p = 0.000 and p = 0.009 respectively). BMI was directly related to CIPP (r = 0.41; p = 0.000). Vaginal deliveries (r = -0.15, p = 0.08) and diabetes (r = 0.15, p = 0.016) were also directly related to CIPP on univariate analysis. Only smoking status (p = 0.000) and BMI (p = 0.000) were independently significantly related to CIPP on multivariate analysis. CONCLUSIONS: Obesity and smoking showed increased CIPP (maximal Intravesical Peak Pressures generated by Cough). While reduced BMI is related to lower CIPP, smoking cessation does not appear to diminish CIPP. These findings suggest that weight loss may reduce incontinence by CIPP modulation. However, the benefits of smoking cessation without additional lifestyle modification, may have no benefit to improve urinary incontinence.

ABSTRACT

As doenças respiratórias na infância representam importante problema de saúde pública, gerando grande demanda aos serviços médicos. O objetivo desta revisão é analisar as principais condições clínicas na infância que cursam com tosse, condição que frequentemente está presente nas afecções respiratórias. São discutidas as indicações terapêuticas para a tosse produtiva e seca. Como resultado, salienta-se a multiplicidade de quadros etiológicos responsáveis, bem como fatores de risco para o incremento da incidência dessas afecções e se discute a melhor conduta diante do sintoma tosse. Concluindo, o autor salienta a necessidade de esclarecer o quadro clínico, a fim de melhor abordar terapeuticamente a referida condição.

ABSTRACT

One case of transdiafragmatic intercostal hernia after intense coughing fit followed by rib fractures in patient with history of pneumonia is presented. He had a severe coughing fit, developed a right toracoabdominal hematoma and then a tumor that was gradually enlarging. Image exams confirmed the diagnosis. Treatment consisted of surgical repair with the use of a polypropylene prosthetic mesh. It is a rare type of hernia. Only four cases were found in literature. The sooner the disease is diagnosed and treated the better the prognoses will be since it will prevent hernia from strangulation and incarceration.

ABSTRACT

VDVB, 38 anos, masculino, há 5 anos trabalha com jatos de areia. Em julho/2007 paciente referiu dispnéiade instalação súbita, associada à tosse seca, sudorese noturna e astenia. Há 4 meses teve perda ponderal de 10 quilos. Foi internado em franca insuficiência respiratória. Não apresenta comorbidades nem faz uso de medicações. Ao exame físico apresentou taquidispnéia (freqüência respiratória: 40mpm), taquicardia (freqüência cardíaca: 135bpm), tiragens intercostais, estertoração crepitanteem bases pulmonares, diminuição de murmúrios vesiculares à direita e saturação de oxigênio de 86%.Ao RX de tórax havia condensações alveolares difusas bilaterais e de aspecto confluente peri-hilar e basal direito. BAAR negativo. PPD não-reator. Ecocardiograma mostrou tronco da artéria pulmonar e calibre mantidos,fração de ejeção de 59% e prolapso de válvula mitral. À tomografia computadorizada, firmou-se o diagnóstico de pneumoconiose, devido à presença de micronódulosconfluentes formando massas parenquimatosas e subpleurais, com presença de adenomegalia para-hilar;cavitações ausentes. Medicações em uso no momento da internação: atrovent, berotec, meticorten, talofilina.Após compensação da insuficiência respiratória e paciente teve alta hospitalar e foi orientado quanto à irreversibilidade caso e a possibilidade de transplante pulmonar.

ABSTRACT

La tos es un frecuente motivo de consulta en la práctica ambulatoria. Aunque generalmente es autolimitada, cuando es crónica e intensa puede ser causa de complicaciones como síncope, neumotórax o más raramente fracturas costales. Presentamos un paciente con fracturas costales múltiples inducidas por la tos. El diagnóstico fue confirmado por un centellograma solicitado luego de la sospecha clínica debida a la intensidad y persistencia del dolor y a pesar de no mostrar alteraciones en las radiografías convencionales. Un centellograma de control a los 5 meses mostró desaparición de los focos hipercaptantes. Las fracturas costales múltiples son una complicación infrecuente de la tos que debería considerarse cuando el dolor torácico es intenso y persistente aun con radiografía de tórax o parrilla costal normal.


Multiple rib fractures associated with cough.


Cough is a common symptom in ambulatory practice and in general does not require specific treatment. When the cough is chronic it can provoke complications such as syncope, neumothorax or, rarely, rib fractures. We report a case of a patient with multiple rib fractures caused by cough. The diagnosis was performed by scintigraphy but fractures were not found in the x-rays. Five months later, a scintigraphy showed disappearance of the increased uptake areas. Multiple rib fractures are a cough complication which should be considered when the patient’s pain is persistent even if there is no x-ray evidence.

ABSTRACT

Migration of an implantable port catheter tip is one of the well-known complications of this procedure, but the etiology of this problem is not clear. We describe here a case of migration of the tip of a port catheter from the right atrium to the right axillary vein in a patient with severe cough. Coughing was suggested for this case as the cause of the catheter tip migration. We corrected the position of the catheter tip via transfemoral snaring.

Intercostal transdiaphragmatic hernia is a rarely reported lesion. Trauma is the commonest cause. We report a 75-year-old man who presented with transdiaphragmatic intercostal hernia due to chronic cough. He recovered after surgical repair.

ABSTRACT

Air flowing through a pipe exerts frictional stress on the walls of the pipe. Frictional stress of more than 40 N/m2 (velocity equivalent of air 113 m/s) is known to cause acute endothelial damage in blood vessels. The frictional stress in airways during coughing may be much greater, however, since the velocity of air may be as high as speed of sound in air. We suggest that high levels of frictional stress perpetuate airway inflammation in airways which are already inflamed and vulnerable to frictional stress-induced trauma in patients with asthma. Activities associated with rapid ventilation and higher frictional stress (e.g. exercise, hyperventilation, coughing, sneezing and laughing) cause asthma to worsen whilst activities that reduce frictional stress (Yoga 'Pranayama', breathing a helium-oxygen mixture and nasal continuous positive airway pressure) are beneficial. Therefore control of cough may have anti-inflammatory benefits in patients with asthma.

ABSTRACT

El despertar suave de una anestesia general frecuentemente se interrumpe por episodios de tos, inducida por la irritación provocada por el tubo endotraqueal. El objetivo de nuestra investigación fue encontrar una técnica para mitigar el reflejo tusígeno y sus efectos indeseables, observando la influencia que la lidocaína depositada dentro del manguito tiene en la tolerancia al tubo endotraqueal. Fueron estudiados sesenta pacientes, entre 20 y 40 años, ASA I-II, bajo anestesia general reglada y con un tiempo de intubación igual o mayor a los 120 minutos. Los pacientes fueron asignados aleatoriamente a uno de tres grupos de 20 pacientes cada uno: un Grupo C en el que se utilizó agua destilada, otro Grupo T con lidocaína al 4 por ciento y un Grupo S con lidocaína al 10 por ciento. Tuvimos especial cuidado en chequear la indemnidad del manguito previo y posterior a su uso. La evaluación de los grupos se realizó utilizando una escala que otorgaba menor o mayor puntaje según la tolerancia al tubo endotraqueal. Después de comparar a los grupos de a pares, encontramos diferencias estadísticas significativas (p<0,005), concluyendo que la lidocaína intramanguito actúa como un reservorio de anestésico local que influye sobre la mucosa traqueal adyacente, brindando al paciente una mayor tolerancia al mismo y mitigando así el reflejo tusígeno. La técnica utilizada en el presente ensayo probó ser sencilla, segura y económica, indicada en todo paciente en quien el reflejo tusígeno puede ser un factor complicante.

ABSTRACT

Over a period of five years 110 cases of incisional hernia were evaluated at teaching hospitals of Dow Medical College and Peoples Medical College, Nawabshah to ascertain their causes. It was found that these hernias were seen more frequently after emergency surgery [87.27%] performed by resident surgeons [89.09%]. About 70% patients had previous history of gynaecological and obstetric operations. Midline incision is more prone to herniation than paramedic and transverse incisions as evidenced by 73.64% patients in this series. Other important causative factors are wound infection [69.09%], chest infection [30.91%], abdominal distension [14.55%] and constipation
